Synopsis

Foundations of Clinical Pharmacy Practice is an academic book that provides Pharm.D learners with the necessary knowledge to enhance their diversity of understanding about the fundamental principles, activities, and the changing role of clinical pharmacists in medical care. The book recognizes the process of moving towards traditional dispensing to the patient-centered approach with focus on the evidence-based pharmacotherapy, appropriately managed medication therapies, adverse drug reactions, and effective patient counseling. It will balance some theoretical information with practical application to prepare the students to make rational therapeutic choices, work in multidisciplinary teams, and create better outcomes in patients. Divided into five fundamental chapters, the book can be viewed as a systematic guide and a reference tool because it is likely that the upcoming pharmacists will possess the skills they will require to address the needs of contemporary clinical practice.
